DuPage Animal Friends MVP Sweepstakes Full Official Rules
The DuPage Animal Friends Tackles MVP Sweepstakes (“Sweepstakes”) is being conducted by DuPage Animal Friends (“DAF”), a tax exempt 501(c) charitable organization. The Sweepstakes begins November 6, 2020 at 12:00 am CT and ends December 4, 2020 at 11:59 pm CT (“Sweepstakes Entry Period”). By entering the Sweepstakes (“Entry”), the participant acknowledges that he or she is aware of, and agrees to be bound by, these Official Rules. Participant also agrees that the decisions of DAF are final on all matters relating to the Sweepstakes, and releases its respective directors, officers, employees, sponsors, representatives and agents from all liability, loss, or damage arising out of their participation in the Sweepstakes or with respect to the awarding, receipt, possession, use and/or misuse of the prize. DAF reserves the right to cancel the Sweepstakes or change any element thereof, with or without notice, at any time, due to errors or other mistakes in the online or offline production of the Sweepstakes.
Eligibility
Any individual who is a legal resident of the 48 contiguous United States (including District of Columbia) who is 18 (eighteen) years of age or older at the time of their Sweepstakes entry. All federal, state, and local laws and regulations apply. DAF Board members and their immediate family are not eligible to participate. The Sweepstakes is void where prohibited by law.
How to Enter
Online at dupageanimalfriends.org when casting at least one vote for their favorite MVP (Most Valuable Pet) and donating at least $5.00 (five dollars) on the Sweepstakes promotional donation page or donating in any amount to DuPage Animal Friends during the Sweepstakes Entry Period.
By mail, when sending an entry on a 3” x 5” card with entrants first and last name, home mailing street address, telephone number and email address to DuPage Animal Friends Tackles Homelessness Sweepstakes, PO Box 3221, Glen Ellyn, IL 60138. Entries must be postmarked by November 4, 2020 and received by November 11, 2020. One envelope equals one entry.
There is no limit to the number of entries per person. DAF nor its directors, officers, employees, sponsors, representatives, and agents assume any responsibility or liability for lost, late, misdirected, mutilated, incomplete, illegible, or undelivered entries, or any other failure to receive a sweepstakes entry.
Prize Drawing and Odds
Prize drawings will be conducted by a representative of DAF on December 14, 2020. The potential winning name is selected at random from all eligible entries. The odds of winning are based upon the number of eligible entries received.
Prize Description
Winner will receive the football helmet autographed by Jay Cutler along with a Certificate of Authenticity signed by a Chicago Bears Representative and dated 4/2/2013. Approximate retail value: $250.00 (two-hundred and fifty dollars). The value of prize was estimated based on online analysis of autographed Sports Memorabilia at the time of Sweepstakes. DAF is not responsible for any change in value. The prize will not be substituted or exchanged for cash.
Notification of Winner
The Prize Winner will be notified according to the contact information provided to DAF at the time of entry and is subject to verification using a valid form of identification. If the winner cannot be located within five (5) business days following the drawing, the winner will be disqualified, and DAF will select another potential winner through another random draw. Winner is required to agree to the terms and conditions of the prize and agrees to release DAF and its respective directors, officers, employees, sponsors, representatives and agents from all liability, loss, or damage arising out of the awarding, receipt, possession, use and/or misuse of the prize. Winner also agrees that DAF may use the entrant’s name, photograph, likeness, statements, biographical information, voice and city of residence for promotional or advertising purposes in connection with the Sweepstakes in all forms of media, now or hereafter known, in perpetuity, without review, permission or further compensation, except where prohibited by law. Winner will be issued at W-2G.

A copy of these Official Rules and the winner is available on December 14, 2020 by sending a self-addressed stamped envelope to DuPage Animal Friends Sweepstakes, PO Box 3221, Glen Ellyn, IL 60138.
The Chicago Bears and Jay Cutler are not sponsors of this Sweepstakes.
